But most menus items are not and I get a white square and a wheel.

Can you try running FoldingText from your Guest user account and see if that works?
I expect that will. If so it means there’s likely to be something messed up in FoldingText’s settings (in your account). You can try to clear and reinstall FoldingText using a tool like AppCleaner.
Did that. Now works perfectly.
